Completing and
Returning the FBI NICS E-Check Enrollment Form
Every FFL can access Enrollment instructions online at http://www.fbi.gov/nics-ffl,
which will include a link to the FBI NICS E-Check FFL Enrollment form.
This form must be filled out, signed, and returned to the NICS Section by fax at (877) 274-0003, e-mail to nics_upeg@fbi.gov, or by mailed to:

| Only the original administrative user (Primary User/Manager or Org Lead/Owner) needs to submit the enrollment form to the FBI NICS E-Check.
The returned form must contain both the signature of the applying user and a witness. For a list and description of each user role, see the
User Role section immediately after the
FBI NICS E-Check Enrollment Form section.

Access to the FBI NICS E-Check will not be granted unless
the NICS Section receives a signed copy by
fax, e-mail or mail of the enrollment form.
